There are legal organizations dedicated to lobbying for laws to make trans people's lives easier, medical and therapeutic groups who tailor their treatment to trans needs, and many transgender individuals who make themselves available to those who could use some guidance in navigating transition.
Lamba Legal
Lamba Legal provides pro bono legal services to help trans people overcome legal boundaries to transitioning. They have many resources for trans people, such as this map of which US states allow you to designate your gender as X on a driver's license.
